Cancellation and Reservation policy

We require two business days’ notice to cancel or reschedule. A valid credit card is required to book any medical, medical aesthetic, or skincare appointment and will be kept on file. We charge a $50 fee if two business days’ notice is not received. We reserve the right to decline appointments if arrival is more than 15 minutes late.

Reservation Policy for Dr. Spencer

We require two business days’ notice to cancel or reschedule. A valid credit card is required to book a consultation with Dr. Spencer. A $75 consultation fee is due at the time of service for the following:

  • Consultations for new patients
  • Consultations for established patients seen more than one year prior to their booking date
  • All cosmetic injectable appointments
    • The $75 consultation fee serves as a credit only for cosmetic injectable treatments done that day.

Reservation Policy for Nurse Injectors, Laser Nurses, and Estheticians

A valid credit card must be kept on file to reserve your appointment. We must receive two business days’ notice to cancel or reschedule, or a $50 fee will be charged to the card on file. We reserve the right to decline appointments if arrival is more than 15 minutes late. Fees are PER appointment scheduled.

Appointment Confirmations

Patients must confirm their medical, medical aesthetic, and/or spa appointment no later than two business days prior to their appointment. Failure to confirm your appointment may result in forfeiture of your appointment and your reservation fee. A cancellation fee of $50 will be assessed as well to the credit card on file. It is the patient’s responsibility to keep record of their appointment dates and times.

Repeated No-Shows/Late Reschedule/Late Arrival Policy

After two no-shows, late reschedules, or late arrivals, you may be required to pre-pay $100 for your medical, medical aesthetic, or skincare service to schedule again. Pre-payments are non-refundable.

Timing Considerations for Consultations

When booking a consultation for a cosmetic injectable treatment or a skincare/spa service, you must specify if you want a consultation only or if you are considering having treatment on the same day. This allows us to book the appropriate time needed for your potential treatment/service.

Surgery Deposit, Physician Fee, and Surgery Cancellations

A $500 non-refundable deposit is required to schedule surgery. This non-refundable deposit will be deducted from your surgical fee. The payment for the remaining balance must be made no later than your pre-operative appointment. We accept cash, check, credit card, and Care Credit. If you need to cancel your procedure, we need a minimum of two weeks’ notice. Due to the length of procedures, difficulty filling last minute appointments and rescheduling all necessary providers, procedures that are rescheduled with less than two weeks’ notice will forfeit the $500 deposit. Another deposit will be collected prior to rescheduling. If your procedure was scheduled to take place in our office under IV sedation, you will also owe a late cancellation fee of $500 to Brandi Lane MSNA, CRNA. This must be paid before you reschedule. After three cancellations or reschedules, a full non-refundable surgery payment will be required before scheduling a new procedure date.

Surgery Quotes

The physician fee quote includes your surgery, your pre-operative visit, your post-operative visit, and our customized product package for you to use to protect your investment. Prescription medications, surgery center fees (for procedures taking place at North Hills Surgery Center or Northwest Springdale Hospital Jones Clinic), and anesthesia fees are separate expenses. Quote sheets are provided at your consultation with Dr. Spencer and are valid for one year. If one year passes and you have not had your procedure, you will need to schedule another consultation with Dr. Spencer and will receive a new quote sheet.

Unpaid Balances

Payment is due at time for service for all appointment types. If there is an unpaid balance, patient will have 30 days to pay the balance. If the balance is unpaid after 30 days, it will be charged to the card on file.

We offer many procedures under local anesthesia that would historically be performed under general anesthesia, saving many of the risks, costs, side effects of general anesthesia (such as nausea), and recovery from general anesthesia.
